The SARS-CoV-2 variant, Omicron, shows rapid replication in human primary nasal epithelial cultures and efficiently uses the endosomal route of entry
Data on Omicron’s global prevalence indicates this variant is one of the most rapidly spreading SARS-CoV-2 variants observed to date. Increased reinfection rates and vaccine breakthroughs undoubtably contribute to this rapid growth, but other factors such as an inherent increase in transmissibility could also be playing a part. Here we show that Omicron virus replicates extremely rapidly within primary cultures of nasal epithelial cells, even more so than Delta, which itself replicated faster than any previously characterised variants. We propose that Omicron achieved this rapid replication rate by becoming less specialised in terms of its cellular tropism. Earlier SARS-CoV-2 isolates were limited in their tropism by their requirement for cell surface entry using the TMPRSS2 protease, and it appears subsequent variants, such as Alpha and Delta, became even more heavily reliant. However only a low proportion of cells in the upper respiratory tract express both ACE2 and TMPRSS2.
Here we show that Omicron has adapted to additionally use the ubiquitous endosomal pathway for entry, thus expanding the number of cell types it can infect, as single cell RNA seq data indicates that seven times more cells in the nose express both ACE2 and the ubiquitous endosomal cathepsin proteases than co-express ACE2 and TMPRSS2. Unlike previous SARS-CoV-2 mutants lacking the furin cleavage site, which used the endosomal pathway but were heavily restricted by endosomal restriction factors, such as IFITM proteins, Omicron appears refractory to this inhibition.
Whilst the switch in entry pathway has increased its inherent transmissibility, the less efficient use of cell surface TMPRSS2 by Omicron Spike protein has also resulted in a decrease in the propensity for syncytia formation, as we and others have now shown. This in turn may decrease disease severity since syncytia have been reported in autopsy of COVID cases and the efficient cleavage at the furin site that underlies syncytia formation has been associated with enhanced disease severity in animal models. Moreover, the contrast between the dominance of Delta virus in Calu-3 cells derived from a bronchial adenocarcinoma, vs Omicron virus in primary nasal airway cells suggests the Omicron variant may be more likely to replicate at the site of transmission and less so in the deeper lung, thus driving the virus towards efficient transmission and away from severe disease, as is now being seen in some animal models.
In this study we found that the Omicron Spike protein resulted in a high ACE2 binding in the context of whole Spike protein, consistent with several other studies, that may also contribute to increased transmission. In addition, we found Omicron Spike bound better to mouse ACE2 than any previous variant, as several others have reported, as well as to the ACE2 proteins of some domestic avian species, and certain bat species. This leads to the possibility that the host range of Omicron may be widened even further giving it the potential for reverse zoonosis and establishment of novel wild or domestic animal reservoirs from which future SARS-CoV-2 variants or recombinants might emerge. However, whether ACE2 is the only species-specific factor that currently limits host range potential is not clear, for example we were previously unable to infect avian cells, even when transfected to express human ACE2. This data may also give some tentative clues about any potential animal origins of Omicron which remains a popular hypothesis.
In summary the Omicron Variant of Concern demonstrates important biological changes compared to previous SARS-CoV-2 variants that may be leading to a more transmissible, but less inherently severe virus. Any decrease in inherent severity should be considered in the context of the extremely high cases numbers resulting from widespread transmission, which will lead inevitably to high health care burden as vulnerable individuals succumb. The emergence of Omicron variant underscores the continued need for vaccination strategies to keep pace with the virus evolution to limit spread and disease severity. Moreover, Omicron demonstrates high potential to establish a reservoir in other animals, and this makes it even less likely that SARS-CoV-2 will ever be eradicated. |

After 2 doses of COVID-19 vaccine, vaccine effectiveness against Delta infection declined steadily over time but recovered to 93% (95%CI, 92-94%) ≥7 days after receiving an mRNA vaccine for the third dose. In contrast, receipt of 2 doses of COVID-19 vaccines was not protective against Omicron. Vaccine effectiveness against Omicron was 37% (95%CI, 19-50%) ≥7 days after receiving an mRNA vaccine for the third dose. |

PS. Хотя нет, в Дании аж до -76% для Пфайзера после 3 месяцев опускалось, это для Модерны лишь -39%.
Интересно, сколько в США, если исключить пару штатов, где сделали qr-коды. В принципе поведение и без них, просто из-за факта вакцинации, изменилось. |

Hospitals see big jumps in COVID-19 patients, but this surge is different from last winter
Roughly two-thirds of patients who have tested positive at hospitals run by the L.A. County Department of Health Services were admitted for something other than the coronavirus, according to Health Services Director Dr. Christina Ghaly. That is starkly different from what the county’s public hospitals saw in earlier surges, when most coronavirus-positive patients were hospitalized because they had been sickened by the virus, Ghaly said.
This time around, many “may have not known they were COVID-positive ... but they’re in the hospital for something else,” Ghaly said.
During last winter’s surge, more than 80% of COVID-19 patients were in the hospital because they were experiencing severe illness associated with the disease, according to the Department of Health Services. |

Canadian passengers stranded after party flight to Mexico
Passengers who filmed themselves partying without masks aboard a chartered flight from Montreal to Mexico face being stranded after three airlines refused to fly them home to Canada.
Sunwing Airlines cancelled the return charter flight from Cancun that had been scheduled for Wednesday and Air Transat and Air Canada also both said they will refuse to carry the passengers.
Adding insult to injury, they were branded “idiots” Wednesday by Prime Minister Justin Trudeau.
Air Transat said on Twitter that the “disruptive passengers” from the Sunwing flight had been trying to return home on its flights, but were denied boarding based on the company’s obligation to ensure passenger and crew safety.
Air Canada issued a statement Wednesday that “to the extent that we can identify the passengers who were part of the group, Air Canada is denying boarding to ensure the safety of other passengers and its crew.” |
Among those looking for a way home was Rebecca St-Pierre, a 19-year-old student from Trois-Rivieres, Quebec, who said she felt abandoned, not knowing how she’ll pay for her hotel room as her stay is extended indefinitely.
She said she tested positive for COVID-19 on Wednesday and was in isolation in Tulum, south of Cancun. She estimated that about 30 others on the flight have tested positive.
“The organizer just left everybody. I don’t know who’s still here. All the flights have been cancelled,” an emotional St-Pierre told The Canadian Press.
St-Pierre acknowledged that the videos give an accurate picture of what went on during the five-hour flight to Cancun.
St-Pierre said that ahead of the scheduled return trip, some people were putting Vaseline up their noses in an attempt to thwart COVID-19 testing. |

We estimated effectiveness of prior infection in preventing reinfection (PES) with Omicron and other SARS-CoV-2 variants in Qatar.
PES against symptomatic reinfection was estimated at 90.2% (95% CI: 60.2-97.6) for Alpha, 84.8% (95% CI: 74.5-91.0) for Beta, 92.0% (95% CI: 87.9-94.7) for Delta, and 56.0% (95% CI: 50.6-60.9) for Omicron. Only 1 Alpha, 2 Beta, 0 Delta, and 2 Omicron reinfections progressed to severe COVID-19. None progressed to critical or fatal COVID-19. PES against hospitalization or death due to reinfection was estimated at 69.4% (95% CI: -143.6-96.2) for Alpha, 88.0% (95% CI: 50.7-97.1) for Beta, 100% (95% CI: 43.3-99.8) for Delta, and 87.8% (95% CI: 47.5-97.1) for Omicron. |

Странная история случилась с нашим знакомым из Израиля.
Ему 74 года, он все ещё спортсмен мирового уровня, бегает.
В декабре мы с женой сидели в ресторане напротив него и его подруги
и весь вечер мы дышали друг на друга. Мы привиты три раза и они три раза.
У нас все прошло незаметно, а у него через два дня сильный насморк,
который потом протестировался как ковид. В дальнейшем все его
окружение тоже подхватило насморк, но ковид у них не выявился… |

Два дня - маловато для инкубационного периода, то есть он скорее всего заразился не в ресторане, а раньше, а в ресторане, возможно, был ещё не заразен. У нас, если делают contact tracing (отслеживая тех, кого ты мог заразить в латентной фазе), обычно интересуются контактами за день до появления симптомов.
Цикл жизни вируса от клетки до клетки - как раз примерно день, а вирусная нагрузка до появления симптомов за день растёт примерно на два порядка. |

Ну штаты здесь выступили слабо.
Я говорю об известных людях, которым медецина доступна. Специально я не слежу, но новости читаю. Помню в самом начале два высоких начальника "Энергии" съездили на Байконур и померли. И с тех пор регулярно то депутат, то генерал, то другие селебрити.

Не только генералы ж мрут, уже 0.7% населения в стране вечной охоты. Среди пенсионеров смертность должна быть на порядок выше. А генералы-депутаты-гендиректора просто не успели на пенсию вовремя выйти, отсюда и эффект.
Доступ к более качественной медицине не спасает, потому что авось традиционно сильный на всех уровнях.
Депутаты - это вообще отдельная тема. |
